School Health Exam Requirements for the School Year 2019-2020

Preschool

Kindergarten

Physical Exam (dated September 1 of previous school year or later):
blood pressure, BMI, lead screening, diabetes screening, developmental screening
Immunizations

• DTaP 4 or more doses with last dose given on or afterthe 4th birthday
• Polio 4 or more doses with last dose given on or after the 4th birthday
• MMR 2 doses with first dose given on or after the 1st birthday
• Varicella 2 doses with first dose given on or after the 1st birthday

Proof of the required exam and immunizations must be submitted to the school no later than October 15 of the current school year.

Dental Exam by a dentist (dated November 15 of previous school year or later)

Proof of the required exam must be submitted to the school no later than May 15 of the current school year.

Vision Exam by an eye doctor (dated September 1 of previous school year or later)

Proof of the required exam must be submitted to the school no later than October 15 of the current school year.

2nd Grade

Dental Exam by a dentist (dated November 15 of previous school year or later)
Proof of the required exam must be submitted to the school no later than May 15 of the current school year.

6th Grade

Physical Exam (dated September 1 of previous school year or later):
Blood Pressure, BMI, Diabetes Screening
Immunizations:

• DTaP 4 or more doses with last dose given on or after the 4th birthday
• Tdap 1 dose
• Polio 4 or more doses with last dose given on or after the 4th birthday
• Hepatits B 3 doses
• MMR 2 doses with first dose given on or after the 1st birthday
• Varicella 2 doses with first dose given on or after the 1st birthday
• Meningitis 1 dose given on or after the 11th birthday

 Proof of the required exam and immunizations must be submitted to the school no later than October 15 of the current school year.

Dental Exam by a dentist (dated 11/15 of previous school year or later)

Proof of the required exam must be submitted to the school no later than October 15 of the current school year.

New Students (who have not previously attended an Illinois school):

Physical Exam (dated within 1 year of start date)

Blood Pressure, BMI, Diabetes Screening
Immunizations

Proof of the required exam must be submitted to the school no later than October 15 of the current school year.

Vision Exam by an eye doctor (dated within 1 year prior to start date)

Proof of the required exam must be submitted to the school no later than October 15 of the current school year.

Athletes who want to compete in interscholastic sports:

Sports Physical (dated within 1 year prior to end of sport season)

If competing in multiple sports during school year, 1 Sports Physical required.
